WHAT SHOULD I BRING?

• Health insurance card
• Accident form(s)/number if available
• Invalidity certificate(s) if available
• Recent imaging (X-rays, MRI, CT, photographs, relevant medical reports)
• Medication list

For your first consultation please send us by email or fax the completed New Patient Form, or bring it with you if this is not possible

If you wish to complete the New Patient Form in our office, please come ten minutes earlier than your scheduled appointment.
